According to the findings released by the Greenville County Coroner's Office in South Carolina, Panettiere died from the combined toxic effects of fentanyl and several other substances.

Fentanyl is an extremely potent synthetic opioid, while 4-ANPP is primarily associated with its manufacture. Alprazolam is better known by its brand name, Xanax. Methocarbamol is a muscle relaxant, while quetiapine is an antipsychotic medication used to treat certain psychiatric disorders.

Authorities have officially ruled the actress's death accidental. The medical examiner's report revealed no signs of trauma that contributed to her death.

An emergency response that failed to save her

The tragedy occurred on August 16, 2026, in Greenville, South Carolina. Hayden Panettiere was temporarily staying at a residence when a 911 call was made around 1:51 p.m. to report a cardiac arrest.

Upon arrival, first responders found the actress unconscious and began advanced resuscitation efforts. Despite their efforts, she could not be saved. She was officially pronounced dead at 2:32 p.m. She was 36 years old.

In the days following her death, the Greenville Police Department had already indicated that the preliminary investigation revealed no signs of foul play or suspicious circumstances.
